BS32 4QJDear Sir/Madam
Orange associates my name with two account numbers: 57542748 and 54408568. I am particularly interested in information Orange holds about me in connection with account number 57542748.
Can I therefore under section 7(1) of the 1998 Data Protection Act make a Subject Access Request and require a copy of all the information that Orange holds about me. This should include explanation as to how any automated decisions about me have been made.
Specifically please include
- any copies you may have kept of my personal identification documents (including but not restricted to debit card, drivers licence, signature on contract) associated with any account number
- a copy of any signed contracts associated with any account number
- any copy you may have of any CCTV footage which you associate with my personal details dating from 5 December 2008 and taken in the independent Orange Shop 261 Oxford Street
- any other personally identifiable correspondence, emails etc which make reference to me in connection with account 57542748
- any instructions Orange may have issued to debt collection agencies in connection with me
- any advice or notification Orange has ever given to credit reference agencies about me
- a full list of the communications data Orange associates with account 57542748 and my name
- complete details of the location data from the handset associated with the account number 57542748 at all times between 5 Dec 2008 and 28 February 2009The Act requires that you respond within 40 days but I’d be grateful if you were able to respond as soon as possible. It’s fine to respond either on paper or in standard electronic format such as pdf, jpg or any MS Office or OpenOffice compatible documents. If you have any queries please reply by email or call me on my Orange phone 07973 115024 (but not 13-18 May).
I enclose a cheque for £10 towards your costs.
Yours faithfully
Does that cover everything? Is any of it ambiguous? Could it be better addressed? Is the tenner a bit on the generous side (what are SAR chrge levels nowadays anyway?)

Perhaps you should send a copy to your friendly not-quite-friend Jaime Peart. On an optimistic view, it may dawn on them that you are behaving like one of those people who stays in prison far longer than they need to because they refuse to admit guilt for a crime they didn’t commit, despite the huge practical benefits which would come from going along with the system. On a pessimistic view, you may have to keep at it for 27 years…
Send it recorded delivery so that you can start the clock running on the 40 days…
LOL! “To assist in your search for the CCTV pictures I enclose a photo of myself. I am a 6′9″ white male around 50 years old, unshaven and bespectacled.” I like it.
