And another one for the poor old Orange shop whose inadequate client ID procedures have caused me all this grief, hassle and expense:

I write in connection with Orange account number 57542748.
I wish, under section 7(1) of the 1998 Data Protection Act, to make a Subject Access Request. This means I require a copy of all the information that your organisation holds about me. This should include explanation as to how any automated decisions about me have been made.
Specifically please include
- any copies you have kept of my personal identification documents (eg debit card, drivers licence, signature on contract)
- a copy of any contract you believe you hold with me
- any CCTV footage which you associate with my personal details from the date 5 December 2008
- any other personally identifiable correspondence, emails etcThe Act requires that you respond within 40 days but I’d be grateful if you were able to respond as soon as possible.
Should you require a fee from me then contact me as soon as possible.

You could always enclose a picture of you to simplify their CCTV footage search. That should cause them some confusion
